| Activities are planned in advance based on the number of registrants therefore full refunds cannot be made. Refunds for cancellations of any conference activities (including Trainings, Institutes and CMTE courses) are available by written request only and must be sent to the AMTA national office. 75% of fees will be refunded if the request is postmarked and/or received no later than 10/2/09, 50% if the request is postmarked and/or received 10/3/09-11/6/09. NO REFUNDS will be made after 11/6/09, so please plan ahead. Refunds are processed 45 days after the conclusion of the conference. Refunds requested after October 3, 2009, must be requested by contacting the AMTA national office. | ||

Each scholar will receive: $500 cash for conference expenses at the upcoming AMTA National Conference, Free conference registration, Free participation in CMTE courses and Institutes. Eligibility: Open to current, Professional AMTA members in good standing. Application Instructions:
Submit electronically to: AMTA National Office at scholarships@musictherapy.org. All entries must be received by 6pm EST June 26, 2009. Application must be submitted electronically as a Microsoft Word document, using the official Scholarship Application Form. Click here for the Scholarship Application Form. Additional materials may be submitted in MS Word or .pdf files. If you are unable to send any of the required materials electronically, please contact Scholarship Coordinator, Rebecca Smith (smithr@musictherapy.org) for further instructions. Only complete applications that follow the application guidelines and format will be considered. Please include the name of the scholarship that you are applying for and the application year in the subject line. DEADLINE FOR PROPOSAL SUBMISSIONS EXTENDED to APRIL 17, 2009 Proposal submissions will be totally electronic for the 11th Annual American Music Therapy Association Conference in San Diego, California. Save paper and mailing costs! Submissions are accepted in MSWord format only using this form. Please download/save this form to your computer, then save it as a new file, using the last and first names of the contact person. If submitting more than one proposal under the same contact person’s name, please add a number at the end of the file name to distinguish multiple proposals. Example: If contact person is John Smith, filename should be smithjohn1.doc; second proposal should be named smithjohn2.doc; etc. To enter required information in the Word document, click on the gray boxes below and type in your text. For check boxes, click to highlight box and then type an “x”. Don’t forget to save your work! Please send only one (1) email that includes the completed proposal (all items, A through U) as an attachment to proposals@musictherapy.org. Information for the blind review will be taken from your submission form so please do not include any identifying data in items A through D. Submissions for the Research Poster Session
AMTA's Research Committee is now entirely electronic and will sponsor a
Research Poster Session on Saturday, November 14, 2009; 1:30-3:30 PM. at
the San Diego National Conference. This session will provide
participants with an opportunity to display their research in exhibition
format. Those interested in having their research considered for the
Research Poster Session should:
1) Submit one copy of a 600-800 word abstract; or a copy of the full report (including an abstract). Regardless of choice each submission must include complete information for judging research quality. 2) Submissions must be received as an e-mail attachment using WORD (12 point in Times New Roman). 3) In the MAIN BODY OF THE E-MAIL include all of the demographic information necessary for listing in the final program including 1) the title of the paper, 2) names(s) of author(s) 3) institutional affiliation(s) and a complete address. 4) On the abstract or full paper INCLUDE ONLY the paper’s title. DO NOT include the name of the author or affiliation. The deadline for submissions is JULY 15, 2009. Please send submissions to: cmadsen@fsu.edu Clifford K.
